Physician's Student Health Form
Shlenker students are required to submit a completed Physician’s Student Health Form annually. Please follow the link below to download the form.
Instructions
- Print the form and bring it to your child’s doctor.
- Ensure the form is:
- Fully completed by the physician
- Includes updated immunization records
- Signed by the physician
- Submit the form in one of the following ways:
- Email: sellerin@shlenker.org
- Mail: The Shlenker School, 5600 N. Braeswood Blvd., Houston, TX 77096
- Fax: 713-270-6114 (Attn: Sharon Ellerin)
Important Notes
- A separate form is required for each child enrolled.
- Students must have a physical exam before starting school.
- If your child had a physical on or after January 1, 2025, a new visit is not required—you may give the form to the physician for completion.
- All immunizations must be up to date per Texas state requirements. This will be strictly enforced.
